These forms are to be filled out with the details provided to you by your child's teacher or school administration. Please read the forms carefully. If you have any questions regarding the insurance requirements for transporting students in private cars, please see the office staff. The district requires a copy of your insurance declarations page as proof that you meet the minimum insurance requirements.

Additionally, all parents / adults riding the bus or chaperoning students on the field trips are required to have a negative TB clearance and digital fingerprinting on file with the MDUSD and the Strandwood school office. See office staff about meeting those requirements.
